Clients can attend without a referral - however they will not be eligible for Medicare rebates.

Mental Health Treatment Plan (MHTP)

If a client is eligible for Mental Health Treatment Plan (MHTP) they will receive Medicare Rebates for up to 10 sessions per calendar year

Medicare Item Numbers: 80110 (in-person) or 91170 (telehealth)

A referral acceptance letter will be sent after the initial consultation

A progress report will be prepared after 6 sessions

Client returns to GP for review after session 6 and PRIOR to session 7

A discharge report is sent on completion

Chronic Condition Management Plan (GPCCMP)

If a client is eligible for Chronic Disease Management (CDM), they may receive Medicare rebates for up to 5 sessions per calendar year. These sessions focus on the management of chronic disease, rather than the treatment of a mental health disorder ( which would be the case with a MHTP).

Please note, First Nations Australians can access up to 10 allied health sessions with a CDM referral.
